Patellar tendinopathy is a common overuse condition seen among athletes, particularly those who participate in jumping sports. The management of this condition has continued to challenge health care professionals as the pathology and risk factors have not been fully elucidated. Based on the current literature and collective clinical wisdom, effective conservative intervention includes relative rest, addressing biomechanical issues, eccentric exercise, stretching, and movement retraining. Other interventions including TFM and counterforce bracing are commonly employed, but have weak or little evidence to support their use. In the case of persistent tendon pain, which interferes with functional activities, injectables or surgery may be indicated. Further research is necessary to advance our understanding of the etiology of tendon pathology and our knowledge about the effectiveness of conservative and surgical interventions.
